I want to correct the

name gender

on my

drivers license birth certificate social security card passport

To Change Name On AL Driver License:

*We suggest circumventing Alabama's name change process by finding a state (WA for example) with a less burdensome process. (ex: change your DL to desired state & update name through those local process.)*

 

1. Fill out   PS-12 / Name Change Form

 

2. (Each county in Alabama has different background check requirements, contact your local probate court to find out which form(s) you need) Most require this Background Check Form

 

2.1. Contact Local PD or Sheriff Dept and inquire about setting up an appointment for fingerprinting or receivingFingerprinting Cards.

 

2.2. Mail completed form to:

Criminal Records and Identification Unit

ATTN: Background Checks

P.O. Box 1511

Montgomery, Alabama 36102-1511", 

 

3. Once you have received certified background check, get document notarized.

 

4.   Gather, Bring & File Documents at your local probate court :

-Name Change Form

-Background Check

-2 proofs of residence 

-photo id

-birth certificate

(Fee, ranges $25-$135)

 

5.   Attend hearing.

 

6. Update name with Social Security.

 

7. Go to local DMV with certified copy of court ordered name change & new Social Security Card to update license.

To Correct Name On AL Birth Certificate:

1.Obtain a certified copy of court ordered name-change.

 

2. Fill out form HS-33

 

3. Mail Documents (with applicable fees) to:

Birth Amendments

P.O. Box 5625

To Change Name On Social Security Card:

1. Bring a certified copy of court ordered name change & multiple forms of ID to your local SSA Office.

To Change Name On Passport:

1. Fill out DS82 renewal of passport form & provide official copy of court ordered name change.

  

2. Mail the form to:

To Change Gender On AL Drivers License:

  In order to update Gender Marker on AL Driver License you must present an updated/amended birth certificate.

 

Those born in Alabama:

As per Policy Order 63 & Alabama Code § 22-9A-19, 22-9A-21, Regretfully Alabama requires a 'proof of sex change' to amend birth certificate.

1. After changing name, obtain letter (template) from 'operating surgeon' and proof of name change

2. Fill out HS-33

3. Mail documents to:

Alabama Department of Public Health, Center for Health Statistics, P. O. Box 5625, Montgomery, Alabama 36103-5625

Those born in Florida:

1. Get a note from a doctor that follows this ( template ).

2. Fill out DH 429 Birth Certificate Amendment form & DH 430 Amendment Affidavit

3. Get documents notarized

4. Mail All Following Documents to:

-Doctors Letter

-DH429

-Dh430

-Photo ID 

-Check/Fee

-Name change if applicable

To Change Gender in Social Security System:

*You do not need to provide medical or legal evidence to change your sex designation on Social Securities' records ( see here ).*

 

1. Go to your local SSA office with a valid form of ID and request it be changed. (in some instances it may be helpful to provide an official ID with your desired gender marker (such as passport), but this is in no way mandatory or required.)

To Change Gender On Passport: 

The amazing news is you can self-select whatever gender marker you like when applying (ds11) or renewing (ds82) a passport, "no proof" required.   (It is by far the easiest official ID for transgender individuals to acquire!)
